Change a child’s last name in Manitoba
How do I change my child’s last name on a Manitoba birth certificate?
Changing a registered legal name and correcting an error are different tasks. For a new legal surname, use Manitoba’s child legal-change-of-name application. If the registration has not been submitted yet, correct the registration form before returning it.
Where to begin
- Decide whether you are correcting a recording error or choosing a different legal name after registration. For an error in an already submitted record, contact Vital Statistics for the correction route.
- For a legal name change, complete the Application for a Child Legal Change of Name. Use the parent/guardian branch matching your circumstances; the form specifies consent and notice requirements.
- Gather the child’s birth/status documents, the applying adult’s identity and Manitoba-residence evidence, and applicable custody or guardianship orders. Arrange the required witness for the affidavit and confirmed copies.
- Submit the completed form, supporting documents and payment to Manitoba Vital Statistics by mail, or in person by appointment. Use the submission address and booking information on the official instructions.

What to check for your situation
A parent must not assume sole custody removes every notice requirement. The child form determines which consent and notice sections apply.
Fees, processing times and document rules can change. This page helps you navigate; it does not confirm eligibility or replace the government’s instructions.

Sole custody has a notice-to-other-parent section. For a child born in Canada, include all previously issued birth certificates, with one showing parents’ names. For a child born abroad, provide a citizenship certificate or PR card and a confirmed birth document with parents’ names. The applying adult provides their own birth or citizenship/PR evidence, identity and Manitoba-residence proof. All custody/guardianship orders must be provided. The affidavit must be witnessed by an eligible person. After an approved change, previously issued birth certificates are invalid. If the birth is registered in Manitoba, the registration and certificate reflect the new name.Checked

After registration, correcting errors and legally changing a name are distinct routes. A registered name is the child’s legal name; legal changes use the Change of Name process. For a problem with a submitted birth-registration record, contact Manitoba Vital Statistics using the contact route on the official birth-registration page. Do not use the legal-name-change form as an error-correction form.Checked
