Apply for OHIP and an Ontario health card
How do I apply for OHIP and an Ontario health card?
Ontario's OHIP guide sets out coverage eligibility, application documents and the ServiceOntario application route. It also separates situations such as newborns and people reapplying after time outside Ontario. Read the requirements for your circumstances rather than assuming an address alone establishes coverage. Approval and the health card come through Ontario's official process.
Where to begin
- Read the official OHIP qualification guidance for your situation.
- Use its checklist to identify the required application form and documents.
- Follow the specified ServiceOntario route or the special instructions that apply.
Before you open the official service
Bring the completed OHIP registration form and three different documents to ServiceOntario. The examples below cover common document choices, not every immigration situation.
- Citizenship or eligible status: an original qualifying document, such as a Canadian birth certificate or current Canadian passport (a passport expired for up to five years is also listed).
- Ontario address: a different accepted document, such as a utility bill, monthly bank statement or lease, showing your name and current home address.
- Identity: a third original document bearing your name and signature, such as a credit card or passport.
- Do not use one document for two categories. Photocopies are not accepted; the residency list specifies which printed or digital versions are allowed and which documents must be originals.
- For work permits or other immigration categories, use the corresponding official section and its additional evidence requirements.
Keep your actual documents and account details off this website. Use the official service’s current checklist before applying.
What to check for your situation
This guide does not decide health coverage or immigration eligibility and is not medical advice. Ontario determines coverage and acceptable evidence.
Fees, processing times and document rules can change. This page helps you navigate; it does not confirm eligibility or replace the government’s instructions.
